If you are seeking services for your child, this is a reminder that both parents need to be notified of therapy services. Please discuss this with your therapist.
Please print and fill out the appropriate paperwork in advance and bring it with you, unsigned, or complete intake paperwork on our client portal. We will need the General Packet, as well as the specific intake for the patient’s age. We will witness your signature when you arrive for your appointment. Thank you.
